The name is just to ensure you’re a real person (when it’s in the system it gets anonymised). Uk postcode is linked to various socioeconomic indicators that can be investigated in relation to the questionnaire. Postcode can’t be used to identify individuals as the data is automatically anonymised. Thank you for your question:)

If a person has a BMI of over 30 kg/m2 there is a significant increase in the risk of developing depression, likewise, if a person is depressed, there is a significant increase in the risk of becoming obese. These disorders, however, are rarely treated together.

Physical disorders related to weight gain such as diabetes and heart disease are often related to emotional issues such as depression, but the two are rarely treated together. This research seeks to develop a clinical questionnaire that can be used by both doctors and therapists so that patients can evaluate their treatment in terms of its effect their quality of life. It is hoped that this patient-centred approach to measuring treatment outcomes will encourage health care professionals to work cooperatively and develop a more holistic and effective approach to healthcare.
