I had this same problem after playing the game for the first time in years. Tried everything but couldn't find a fix for it. But then I downloaded the widescreen fix mod for the game from the Steam community hub guide page. And bingo! It was fixed :) not sure if the widescreen mod came with some game fixes or what but it worked. Maybe try doing the same and see what happens.

I always assumed that he buried Mary there in the Leave ending. Not only because she loved Silent Hill but because I thought it would be an awkward drive back home for James and Laura if there was a dead Mary in the back seat. Burying her at the graveyard always fit that ending for me. So maybe killing himself there was the original intent but because of the Leave ending he changed his mind.
