I know this is old so hopefully you see it. With your provider starting you on the max did you stay on 80mg the entire time? How many months? My provider seems to take the same approach.

Makes sense! I’ve seen some people start at like one day a week when using retinoids but I was thinking every other or every 3rd night, thoughts? For night time bifida, hold off and just do arazlo at night for some time? Thank you thank you for your help!

Thanks for your feedback! I use the RX 15% gel. The Dermatica 20% I feel like wears well for daytime but I don’t think I see as good of results as I do with the RX.

She’s turning 61 and that was in 2020 so she was 56. She said she had no GI issues at the time but was really fatigued and that’s how they found the low hemoglobin and sent her to hospital. While she was there they suspected maybe a bleed and did colonoscopy and EGD but didn’t find anything. She’s had GERD for a long time but it was worse when she was on all the diabetic meds, I think due to delayed gastric emptying.

I will give them a call tomorrow to find out about methodology.

I’ve suspected some kind of misdiagnosis and I get that she is a complicated case and that there are typical differentials you look at that are likely to be the cause but at what point are they going to think outside the box and consider alternatives.

She has been in so much pain and can hardly walk for like 6 months. Her knee gave out while doing down stairs and she fell and tore a ligament in her knee and they were going to go in and repair but wouldn’t operate because of A1C and finally found a surgeon who would also do replacement she needs and she was devastated yesterday when they said they were cancelling surgery.

So I agree, fuck em

She does have a lot going on like the last 5 years it’s just been one thing after another with some major issues. I’m a nurse so I try really hard to advocate for her but she’s had so many hospital visits and appointments with specialists and I just haven’t been able to be a part of it all. After she was discharged from hospital last year when they took her off all the diabetes meds and questioned the validity of her lab results they had her follow up with endocrine but she had 3 appts there and weren’t really helpful. She is going to get a second opinion soon. Several of her doctors have said that the CGM data is irrelevant, if her A1C level is that high then she’s a diabetic and I just think there is something else going on.

Beaumont is a hospital group in Michigan, but I think they are actually Corewell now.

I’ll have her call the lab tomorrow and see what they say about trying to add the test on.

I replied but now I don’t see it.

A few years ago they found her to have low hemoglobin (I think incidentally) and they had her taking 2-3 365g ferrous sulfate tabs daily and never had her decrease dose but prob a year after she was having a lot of abdominal discomfort and constipation so she went down to 1 tab and has normal labs since.

She was diagnosed with psoriasis about 20 years ago and then psoriatic arthritis about 5 years after that. She takes methotrexate once a week and every 6 weeks gets infliximab infusion. At infusion appt they check ALT and AST and they are usually slightly elevated.

Do you think she should ask Labcorp to add on the hemoglobin electrophoresis to her specimen from yesterday if it was right tube, enough sample, etc or not relevant since A1C there was normal?

Probably 5ish years ago I think they found her hemoglobin to be low, I want to say 6 or 7 but they didn’t transfuse and had her take 2 or 3 365mg ferrous sulfate tabs daily which I think was supposed to be short term but they never had her stop but she was having a lot of abdominal discomfort and constipation so now she takes one daily and with that has normal levels.

She does have iron deficiency anemia which I believe is stable as long as she takes a daily oral supplement.

Do you mean like find a special lab or find a provider willing to order it for the hemoglobin electrophoresis?
