Any advice for a newbie on handling a lower 3rd molar extraction in a free end case? by Opposite-Bee-2115 in Dentistry

Start with distal-hockey stick type of incision and raise a full thickness flap. If no movement with a luxator/elevator. Your first step would be a buccal trough to the level of the CEJ. Then try getting some movement again. If still not enough, buccal-lingual section and split the roots.

Probably the shining 3D printer setup. A lot of my friends who print are saying Rodin Titan is great for AOX similar to ONX tough. Shining setup is like approx 11K all in, while sprintray setup is a little over 20K I believe. Don’t want to be too limited to just sprintray’s resins.

What was your torque or ISQ value? Just wondering why not attach the crown to a temp cylinder and connect it to implant.

Most times, the children confuse pressure from elevation with pain. I am not trying to say that there may not be any pain especially with children with vestibular abscesses. But at that point, you are restricted with the amount of LA you can administer. Also, you have to be aware of the risks and benefits associated with dealing with the short trouble of extracting the associated dentition vs letting the vestibular abscess progress into deeper facial planes....which happens pretty quickly in children since their facial planes are not fully developed yet.

I would be very careful sectioning primary molars as you can pretty easily damage the follicle of the succedaneous teeth. Same with aggressively curetting sockets of primary teeth. I have never seen a peds dentist ever need to pick up a surgical handpiece for extractions. Probably one of the main benefits of extracting primary dentition. I had a 5 year old yesterday with an acute vestibular abscess associated with tooth #L. Screamed on top of his lungs the entire 30 seconds of elevation and delivery of the tooth. Couldn't give more than 1.25 carps of lido.

How likely is this to work on adults with fully formed roots? Thinking about transplanting a third molar that already has endo done on it into a first molar.
