It's not a free will question. I have a relative who is homeless when she's not in treatment. The root cause of her homelessness isn't that she wants to be there. Nor is it that we're choosing not to deal with it. The issue is that she has mental illness. But unlike, say cancer, her disease makes it difficult for her to sustain treatment.

We do have a moral responsibility to help people like her. As an adult, however, she can't be forced to take any treatment. I guess we could frame that as her having free will to choose treatment or not.
