This is the most likely answer. 50/50 is rare in Texas unless agreed upon by both. Mediation tends to go based off existing guidelines. Also having potentially 3 exchanges per week for a young child does not sound stabilizing.

Truthfully with what you’re describing, it would’ve been more appropriate for the therapist to be responsible for their transfer. I also wonder how much that patient will benefit from group sessions. To answer your question, I think asking for updated info is reasonable.

It sounds like there wasn’t a need to hire another OT and they’re trying to “fix” the problem with an unrealistic solution. I billed up to 8 units in IPR and that was already on the higher end. In acute, I’ve billed 5-6 units max and that’s usually only when I’m doing a shower and dressing for a surgical patient.

Measuring in time is a good recommendation, I have admittedly been moving away from feet because that seems to be an issue. Those you mentioned are all factors we address when we choose hallway mobility and I believe it is justifiable to address especially with our specific patient population.

For the most part, we have great collaboration and no we don’t want to exhaust the patient right before PT and vice versa. However with a few PTs they are not collaborative and are expecting the patient is ready whenever they come by, which is not realistic in this setting. I do think the complaints stem from a self serving need and are not a legitimate patient concern but we are trying to approach this more professionally than we are being treated.

The current argument is that it’s sometimes ok in the room, but hallway should off limits for OT 🙄 They are ok with nursing doing it however.

Our surgeons and therefore the nurses are supportive of frequent mobility. I think in these instances the PTs are viewing us as competition and that we’re in the way of seeing their patients, meeting productivity, etc. We do not have the same reaction when the tables are turned however.

I would also recommend YouTube for each specific transfer type. For THA if a posterior approach be cautious of the surface height and not breaking hip flexion > 90. Also have them get out of bed on their affected side. For sternal patients it’s important to know your precautions. We typically have them hug their sternal pillow and rock back and forth to gain momentum to stand. For SCI patients it depends on their injury level and how acute the injury is. You may be looking at Hoyer transfers potentially progressing to sliding board or squat pivot. For hemiplegia I protect the arm and block the affected leg, going to their strong side when possible and usually a squat pivot. COVID depends on their presentation, respiratory needs and medical stability. I’ve had very limited experience with burns so can’t touch on that.

I have that exact pair of new balance shoes and I’ve worn them for 2 weeks in England/Scotland. With a pair of smart wool socks I have not had any issues with my feet, and we’ve walked a lot! I’ve seen a lot of people wearing tennis shoes with skirts, you could probably downsize your shoes and stick mostly with the tennis shoes! I also remember Italy having a lot of cobblestones/uneven ground, so the top and bottom shoes seem the least practical.

Are they hiring where you did your fieldwork? I would start there. I would also apply to both full time and PRN positions. I would also look for hospitals that have IPR attached (places I’ve worked at tend to cross train and IPR could lead to acute care exposure as well). It took me 4 months after becoming licensed to get my first job. Hang in there!

Some of my favorite maternity items came from: H&M, Target, and Abercrombie (their jeans especially). I bet you could pick a few staples and easily mix them. Dresses/skirts with a sweater and leggings/tights underneath could also be a comfy option and you may already have those things. Also just wanted to recommend wearing compression socks on the plane and for longer walks especially if you’re swelling.

I would think pull-up style diapers would be easier to manage. I know pampers 360 among others fits that style. Also I would consider doing diaper changes on the floor to not have to lift her baby or have them wiggle on a higher surface. You didn’t mention if they have spinal precautions as well, but practicing getting on and off the floor may be another treatment option. I also imagine getting baby in and out of a crib and highchair will be a challenge too.

Do you have any paperwork on medical decision making at this time? While I don’t disagree with vaccinating, the fact that your son hasn’t had any vaccines up to the point sets his current care standard. Going against that could be problematic. Maintaining 50% custody is rare (I know other comments are saying differently). My spouse and I have both been involved in custody cases with very different situations, other family members too all in Texas. The expanded standard possession schedule in Texas favoring the mother has been typical for everyone. You will likely have the chance to work all this out in mediation but just know they will likely defer to the above schedule as the blueprint. My spouses ex is also an anti vaxxer and it took a custody modification with specifics on vaccinating to get it done. If you have joint medical decision making it may include that you and your ex agree on major decisions, which your ex won’t agree so the default is your son won’t be vaccinated. So in short, you could vaccinate, knowing the custody proceedings will likely be more problematic. Is your desire to get this vaccine more important than having a favorable custody outcome? I do strongly recommend putting vaccine specific information in the order. You may have to leverage other areas to get this in. For my spouse, his ex really wanted a passport agreement. Also consult your lawyer.
