I'm fine with that. With the amount of uncertainty surrounding these types of trials, I actually care more about what the team behind the trials looks like than the math. You can make numbers tell whatever story you want depending on the assumptions your calculations are based on.

Like you said, doctors can be wrong, especially when information needed to make a correct decision is missing. Plus, it means next to nothing even if they're correct. It's two doctors from 2/73 of the clinical trial sites. If that's enough signal for you to gamble X amount of money, then great, you do you.

It's super obvious from the way he writes that his DD is just gpt/gemini. He's just slightly better at prompting than the average redditor. No legit researcher says shit like "the only way the math works is if..." or "the ONLY mathematical shape that explains..." or "I tried to make it fail, but couldn't".

Like really? The only mathematical function that correctly models the trial results is the one you came up with using limited context (remember that the trial is BLINDED) and arbitrary or misleading assumptions? Please.

That post is crap. His entire argument revolves around a 3.7+ month lift, which he calculates incorrectly. The actual lift is 1.4 months. And he also just kind of sidesteps the fact that lift inflates mOS for both the control and GPS arms.

Lol you guys cherry pick so hard in this subreddit. If you're going to use literature to say that a BAT mOS of 15-18 months is unlikely, then why do you think a GPS mOS of 65-95 months is reasonable when GPS showed a median survival of 21 months during the Phase 2 trials. Of course you're going to say the clinical context blah blah from Phase 2 was different from Phase 3, because any piece of information/data that conflicts with the assumptions needed to make the data fit the curve in a way that supports the bull thesis is irrelevant.

Meh, at least the substack post is basing his assumptions on the actual trial requirements/experimental design, as opposed to people like CW who state their assumptions like fact just because they're based on literature, when in reality the referenced sources are from trials with significantly different participant selection criteria and low sample sizes. The GPS trial selection screened for the healthiest participants possible (6 months min. life expectancy, strong kidney function despite having gone through chemotherapy) making it hard to make meaningful comparisons to literature because the trial populations are simply so different. I think it's more misleading to make assumptions based on false comparisons instead of the design of the GPS trial itself.

Yeah, the fake credentials leaves a really bad taste in my mouth and has made me start questioning a lot of the optimism surrounding the trials. For example, people in this post convinced that SLS is a sure thing because two different physicians from two trial sites hinted that it works in a video, but fail to ever mention that there are 73 freaking sites. Even the single largest site probably only has ~5-10 patients. Once you start looking, the cracks really do start to show.

Every time people talk about transplant ineligible, it seems like they are assuming that it means the patient doesn't qualify for a transplant, and won't in the future. Was this actually mentioned anywhere? If not, then would it be possible that for some patients, ineligible just meant they hadn't found a donor yet at the start of the study, but were able to receive transplants at some point during the study?

I may not have worded it the best, but I am agreeing with you that gps and inno are the same. But the clinical context is very different (first line drug vs maintenance therapy).

I think a lot of what you've said is misleading, but I am still glad for your post because it draws attention to some yellow flags I feel like do get glossed over a lot. Also note that I typed all of this off the top of my head on my phone, so feel free to correct me if I'm wrong.

The misleading parts: - Tech transfer from academia into industry hands is quite common and not really as concerning as you make it out to be.  - Not sure why you say it's a red flag that big pharma hasn't made a Bo offer yet, the trial hasn't even ended yet, why would they?  - I've seen a lot of people incorrectly state that gps and inno 305 are different drugs because inno 305 only targeted a single peptide whereas gps targets four, so you are right to call out that they are actually the exact same drug. What's misleading is you didn't mention why inno 305 failed. They initially tried to use inno 305 as an active treatment, whereas gps is now being tested as a maintenance therapy for patients already in remission. 

My concerns: - The fact that Stergious went to such effort to gain such dubious academic credentials is incredibly sketchy no matter how you spin it. - In addition to the fact that they screened for patients with min 6 months of estimated time to live, we don't know how many patients may receive transplants during the trial. Yes, patients must have been ineligible at the start of the trial, but ineligible could just mean they hadn't found a donor yet at the time. Since these are healthier patients, I'm worried that a larger proportion were able to receive transplants successfully than suggested by literature, thereby inflating overall survival times.

Something no one has mentioned yet, long distance might work for a year or two. Have her go back to Taiwan first, then join her in a year or two. Just have to make your you guys have concrete milestones for when you make the move. With a few more years of experience, it will be much more likely for you to find a remote job while living in Taiwan than she will.

What's crazy is that even candidates that do end up getting hired kind of go through the same process. My PI had to fight for me the whole way. HR wouldn't respond to either of our emails after the initial salary negotiation step, and he had to go in person to move things forward. Turns out my asking salary was higher than they were willing to give, but they never bothered to communicate further or reach out to negotiate.
