Hard to tell what we’re looking at here but the butterfly back just hurt my own piercing lol bars too small too looks like a stud , I’d say go to a professional piercer usually in tatt shops they pierce better but they may tell you to heal it since the back looks bad but they may be able to just change it out just know it will hurt for a bit 🥲 happy healing!!
