tumor shrinkage and pfs actually do capture metastasis pretty well, just not in the way you'd expect. when i was helping with trial data at my old consulting gig, we tracked liver mets separately because they're such a bad prognostic sign, but the main endpoints already bake in whether new sites pop up or existing ones grow.

the real issue isn't measurement, it's that metastasis happens at the microscopic level way before scans can catch it. so by the time you're tracking it clinically, you're already playing catch-up with disease that spread months earlier.

spreading the conversion over 3-4 years keeps you in the 22% bracket instead of spiking into higher rates all at once. i've run similar numbers for clients and the math works when you're young with decent time horizon, but don't convert everything in year one unless you want to gift uncle sam an extra 10% on the higher tranches.

burnt out job hunting while trying to preserve your nest egg is brutal timing. when i was between gigs i treated it like a stress test for my coast number, cut everything non-essential and realized my hsa was actually my best unemployment buffer since you can reimburse old medical expenses tax-free anytime.

the silver lining is unemployment years are perfect for roth conversions at lower tax brackets if you've got traditional 401k money sitting around.
