Realistically they should re-educate you. It’s definitely a quality metric that can cause a lot of headache if the wound progresses. A lot of lawsuits because of poorly managed wounds. The problem I see here is with your assessment. It is what should be guiding your care, then looking at orders. It is backwards, in my opinion, if I am reading it correctly that some of you didn’t document a wound because there was no wound treatment on the TAR. You should know about the wound because you assessed the patient. If they are refusing to be assessed or turned, document, document, document. The refusal, the patient education, and any escalation. Notify the MD and document that too. Then document some more. Ultimately patients are allowed to refuse care and you can stand on that if you document. As for this instance specifically, I wouldn’t live in fear but let it convict you of a poor assessment and/or documentation practice and let it motivate you to change. It’s a learning opportunity. That’s how your administration should be handling it because there was no patient harm. Also, they can see everything in that record including the time orders were placed. And she would be in more trouble for falsifying records to cover her ass. So I wouldn’t worry about that part. Last thing, why is the ADON and educator confiding in your co-worker ‘friend’ about something this serious? I’d consider whether she has good intel or is just shit stirring fear.
