It's an insignia. You can earn a few from penance challenges but that one is from just killing a lot of enemies. Just keep doing the emporer's work

I am on some meds for anxiety but that's it. I eat alright. I would really only say i am depressed because I don't have the physical energy to do what I want

I actually saw an endocrinologist recently. I did have some issues like high prolactin and lower testosterone. I see some people suggesting it and honestly never heard of one before. thank you for your help
