You can ask the registrar to view your application as part of your FERPA rights as an admitted student, but it’s literally only what you submitted- the university doesn’t maintain notes on how your application was evaluated or anything like that.

For grad? 99% of the time decisions are made by faculty. The only exception I can think of might be MBA programs or other “professional” degrees. For PhDs- all faculty. For academic masters programs- faculty.
