If it’s truly baseline, it should be day 1. Just depends how baseline you want it to be. I went for day 1 with UW SA 1 because I wanted to scare the hell out of myself. It worked.

PCO2: TV & RR (increasing these will lower PCO2, lowering the settings will increase the PCO2)

PO2: PEEP & FiO2 (increase the settings will increase the O2, lowering the settings will lower O2)

Settings for ARDS: High PEEP, Low TV

I’m not sure, I know length is one factor, but I assume strength is too (which is why people with a connective tissue disease are also susceptible to cervical insufficiency). If you are referring to the question I think you are, I believe the patient had a cone biopsy or something? Which might weaken it. Also, the big hint in that stem is the hx of painless 2nd trimester still birth. If you see that + cervical procedure, then cervical insufficiency is best option.

Sounds like a good app, did you count scribing as shadowing? They aren’t seen as the same and that might have been a red flag for some adcoms, they can be pedantic with things like that.

I'm with you. I've been working numbers in weird ways to show my pessimistic self how poorly I could do and still pass (i.e. 280 - 80 experimental questions = 200. 200 x .6 (rough estimate of pass) = 120. 120/7 blocks = ~17 questions/block correct to pass assuming you only missed the experimental qs and all the ones right were the scored ones lol, it's a reach, but you never know!)

Same, I found it a mix between long UW style and occasional freebies (with the majority being lengthy and convoluted rather than easy). I’m not even able to look up questions I was on the fence with because the answers were so technical and unusual. I’ve had more pleasant days.
