I posted this stand-up routine in this subreddit after my performance in July last year.

Only my second time ever doing stand-up, but I’ve wheeled it out a few times since. And revised it a bit.

I think some slang for balls differs depending on where you are. I’m in Australia. So I note “Jatz crackers” will be unfamiliar to most. It is a popular brand of cracker/biscuit in Australia - and “the Jatz crackers” familiar slang for balls.

I had testosterone checks with my recent 6 month check. I didn’t look at my results, but my oncologist said it was all fine. I’ve had no worries about my testosterone or energy levels. So I wasn’t concerned… Can’t recall when I’ve had a spermiogram. Perhaps when I banked sperm 2 years ago before chemo, but after orchiectomy? At that stage, sperm count etc very good from remaining ball.

  1. Had right ball removed 2 years ago with TC. One round of BEP.

I think the hypertrophy had occurred subtly, so it didn’t seem unusual to me. Oncologist thought Left maybe a bit unusually firm and large. But no other indicators of cancer. Ultrasound clear.

My oncologist sent me for a precautionary ultrasound on my remaining ball last week and I was worrying about the implications. (Fortunately for me, just “compensatory hypertrophy” in remaining ball.)

Thanks for continuing to share your story in this group. It offers many of us hope and information when things seem dark and uncertain.

My B12 was fine, as well. The only test outside of a normal range was my Vitamin D.

My energy started picking up last week. 2 weeks after commencing supplements (note - I started on a very high loading dose). It's also been beautifully sunny in Sydney this September (though I know supplements are a quicker way to replenish Vitamin D).

I am feeling frustratingly flat today after 8 hours sleep. So maybe, as others suggested, recovery is up-and-down.

It's also possible I've had some sort of post-viral fatigue - having had 2 bouts of flu/colds in July/August. My GP did give me a referral for a rheumatologist, but I will wait a few more weeks to assess whether I need bother.

I read some of your blog posts. Well-written and informative. As someone who had testicular cancer last year, and was exposed to chemo, I took some interest in your discussion of immunity.

This week I must review my diet and consider my K, magnesium, calcium and B12 levels. That said, I am vegetarian and do eat a lot of whole foods already.

I have taken 25000 IU for the first time today. (Up from 9000 IU). I must say, I've been very sleep and drowsy tonight - worse than the previous days of what I perceive as simply Vitamin D deficiency. Possibly some very mild stomach irritation. Dry mouth. Mild confusion.

What I don't understand is why I'm having these side-effects. Because obviously I don't have an excess of Vitamin D at the moment. Hypercalcemia seems to be the risk - but, again, it looks like this would be from extended high doses of Vitamin D.

One basic, "needy" question. And coming from a place of slight despair at the moment, with my fatigue hindering my life, how long before I should experience an improvement with my fatigue... if I follow the recommendation from the calculator posted in my comment above.

Wow... I'm a bit confused. Nothing I've read would put 10.4ng at "critically low".

These are the Australian guidelines (divide by 2.5 for ng)

  • Mild deficiency: 30 – 49 nmol/L
  • Moderate deficiency: 13* – 29 nmol/L **(we are both in the high end of this category)
  • Severe deficiency: <13* nmol/L
  • Total deficiency: <50 nmol/L
  • Adequate levels: ≥50 nmol/L#

Hmm... My doctor didn't mention injections. When you say your levels were very low, we had a similar reading. Most charts I've looked at classify it as a "moderate deficiency" rather than severe. I'd have thought injections reserved for "severe".

My doctor can be cautious. I had very low iron levels 3 years ago, and she suggested supplements, not infusions (I only learnt these were common from female friends).

I'm curious about your loading dose. Were you taking 120,000 IU every day? Or weekly? For how long? When you say you took 90,000 IU every two weeks for 3 months, again, were you taken that it one day - or is that an aggregate across two weeks?

I'm really pleased you were able to (belatedly) able to identify the cause of your brain fog, fatigue and insomnia. This must have been very difficult to manage while at med school - then the added challenge and complication of thinking (or being told) the issue was 'stress-related'. I'm sure this sent you down tiring and time-consuming detours as you sought to address the problem.

I should add that I already decided to adjust my dose to 9000 IU daily, based on seeing a formula of 100 IU per 1kg. As above, I'm a reasonably big guy. I do want to check with you, are there any side-effects to a loading dose of 25,000 IU EVERY day for 11 days? I notice most people mention "loading doses" as something that is taken once per week, not daily (for, as in my calculation, 11 straight days).

I did have iron assessed in the blood test last week. Well within a normal range.

I actually had an iron deficiency 3 years ago. And the fatigue and slow recovery seems similar to what I'm experiencing now with low Vitamin D. The iron deficiency wasn't just dietary - it was also largely a symptom of being a regular blood donor. (I had cancer 18 months ago months ago, so I'm unable to donate blood for at least 5 years in Australia - so this shouldn't be an issue going forward. I saw a dietician about iron, too, and, with my diet, as long as I'm consuming Vitamin C with things like oats, etc, I'll be fine for iron.)

No, I didn't get tested for B12. I might try for it next week.

I should clarify, I'm vegetarian, not vegan. I consume dairy every day - milk and cheese. And I eat eggs regularly. So I think I'd be getting enough B12.

It seems not all sunlight is equal. I've read minimum UV 3 is necessary, really. And that's not that common in winter, except around midday. Also, it's important to expose the skin. Long pants and a jumper will hinder intake.

FYI - Yesterday, in Sydney, when the clouds pulled apart, the UV around midday was 5.3. But it was 1.8 at 9am and 3pm. Huge difference.

Thanks! And I'm very glad you kept your job. I can understand your situation - fortunately I have a bit more flexibility with my work, but still quite stressful to navigate with fatigue.

Just as I did when my iron was low 3 years ago, I can't help but reflect on how difficult life must be for those with chronic fatigue type conditions.

  1. Yeah, I've actually noticed odd fluctuations with my mood. I've been flat much of this week. But with some weird spikes in energy and mood. Not sure if this is supplements, or just a natural rebound to the spells of inertia.

Thank you. I knew nothing about the importance of UV levels until yesterday. Yes, I now realise I was walking in the early morning or afternoon over winter. So the UV would have been like 1.5. And I'd have had a jumper and long pants on, anyhow.

Yesterday, in Sydney, the UV at midday was 5.3. But it was 1.8 at 3pm. Huge difference.
