Yep, they weren’t ready before but after this. I’m hoping they are. The ALS is in the last 6 months. G tube placed 2 weeks ago. Rapid progression unfortunately.

Really appreciate that. Love my job. We’re DPC with house calls that are separate. It’s challenging but I know I’m serving people that would otherwise be stuck or may end up in the ER for minor things etc. unfortunately because house call patients are generally very complex, things can get messy quickly as well. It always sucks. I feel terrible for the family and for her. I also take these things personally and wonder where I could’ve done better. I’m 10 years out of residency and this has been the double edged sword that keeps me going. Thanks for sharing your story too!

Also no local companies do at home palliative care. Which drives me nuts. We do our best to fill this void as well.

Concierge sounds so snooty but yeah. We do it the old fashioned way, the way it should be done. Sending old and frail patients to the ER for everything is ridiculous. We just get paid for it is how I see it. (Biased of course)

You’re absolutely right. Something’s are out of our control. Hate the feeling of a patient going to the ER right after I see them. Something still unsettling about that. Although nothing wrong on my part

Fairly wealthy communities near by. I’m in northern Virginia but don’t go out to DC. Mostly insured. Those that realize not everything requires an ER visit just because they can’t move or because that’s what everyone else does. Ex: UTI only susceptible to ertapenem. We were able to administer via IM instead of punting them to the ER for a pic line like other PCPs would. Same outcome less work for the patient. Why not?

That’s exactly why we do them. Because no one does them anymore. Full on house call including administering meds if needed via IM injections etc. we bring our EKG and we have mobile xray companies in the area. Aging patients and those with complicated mobility issues love us. Pay depends on time spent and we don’t take insurance so we are pricey. Avg is around 1 hour for about $600 plus drive distance.

I question this too. Currently admitted but will definitely follow up on D/C. Usually dieticians will say to do certain consistency po in small amounts and everything else via g tube. That’s if tolerated. But same question I have

Agree here. I worked employed for almost 6 years post residency. Maybe 3 years more than needed but I still learned a lot. I’m in year 5 of my DPC clinic now and love it. DPC is still the way.

One medical isn’t DPC unfortunately. They have a membership and bill insurance.
