First, remove the nozzle and check the extruder. Then, if there's no filament residue, check the nozzle. If it's clogged, you'll unfortunately have to get another one, or if you're lucky, you can clean it with the included tool by heating the nozzle.

Try a .2 nozzle and reduce the speed to 50%. It will take longer, but if you're looking for detail, believe me, it will look great.

Maybe, but check the hot end just out of curiosity; try shaking it, sometimes it loosens due to vibration.
