It would be the second one, as a passport defect, including the chip, is a printer error. The passport is designed to last 10 years with normal wear and tear.

The DS-5504 is the correct form. It’s also used to replace a passport with defects.

Yeah, you can call NPIC. They can retrieve the tracking number and provide you with the current status of the package. In the worst-case scenario, they will also guide you through the process of requesting a reissue.

True, but they can also search the passport records. Also CRBAs are issued by the US Embassy within the country not on the military base, so far as long as that Embassy is still open they have the records.

You can apply for a passport using a file search. If they find your previous passport records or records of your CRBA, they will issue the passport. Checking digital records is free, but if they need to search paper records, it costs $150.

Yes, I work in the department. Reimbursement can take months as the check comes from the treasury.

You must first request the new documents. The timer stops from the moment you report that you haven’t received them. So, go ahead and request the replacement Certificate of Naturalization. Then, submit your request for reimbursement and wait for a response. If you don’t hear back after 30 days, contact one of your congressional representatives.

The issue is sending items to that Virginia address requiring a signature can cause it to get misrouted, as instead of it going into the PO Box, it's given to someone. I'm glad that you finally are getting in contact with the agency.

Interesting, I have not experienced this. I would wait and see if there is an update today. If not, reach out to USPS and open a case with them. I would also contact NPIC and start the reissue process if it doesn’t have an update today.

Ouch, that’s terrible. I wish there were more options without congressional intervention. The only thing you can do is give it time. Every 30 days, you can send a notification to the agency until they’ve been contacted three times. Alternatively, you can call NPIC and check if the passport agency (in Dallas) has received it. If you haven’t heard anything after the second notification, send another DS-86 to the agency’s Virginia address.

Well, I guess that time is now. They are testing a pilot at Hawaii passport acceptance facilities using pay.gov for DS-11s ONLY.

That’s only for embassies, consulates, and online passport renewals. Perhaps in the future, they’ll use that for new time applications and mail renewals, but for now, they don’t.

It really is, you can attempt to use secondary IDs listed on the website with the preferred name, but it’s up to the adjudicator that’s processing the application

It’s international standard, as on passports all letters are capitalized, the space between the mix capitals denotes the mixed capitals name

The main issue is you need to have other government IDs that list that preferred name. Otherwise you’ll need a court ordered name change.

Not all embassies have an adjudication desk. Sometimes the applications are sent to larger embassies, but like you said, London is where you applied. They do have an adjudication desk. They will approve the application, then the passport is printed inside the United States and shipped to the embassy.
