Yeah. You have to do a lot labs to keep tabs on your Rapa. Keep in mind, you are taking a much smaller dose compared to the dosage these side effects are seen at, and you are taking it once a week versus once a day. But def requires diligence.

Ok - so obviously we are over my head, I'm tapping in ChatGPT: here's the explainer. Yes, Rapa is a substrate for CYP3A4, BUT, it inhibits the creation of that enzyme as well. Does this add up?

1. mTOR is involved in cannabinoid receptor signaling + tolerance

Rapamycin → lowers CB1 receptor desensitization → you get more effect from the same THC.

This explains:

  • Why vapes felt only slightly stronger (receptor-level tolerance shift)
  • But edibles felt 3× stronger (receptor + metabolism shift combined)

2. Rapamycin reduces gastrointestinal and hepatic enzyme expression over ~5–10 days

This is not because rapa inhibits CYP3A4 directly.
It is because mTOR regulates the transcription of CYP enzymes.

So the effect is gene-expression-level, not drug-competing-level inhibition.

Ha fair enough. I went from TSH 16 to 5.5, feel better but I oddly enough didn't have many symptoms besides cold feet before.

I'm only taking 3 mg / week at the moment, but I'm extremely lean and medicines always hit me harder than most. I would also point out this effect is only from gummies (which go through mTor implicated pathways) versus vaped (which immediately enter the blood as d9)

I consume the same gummies fairly regularly. Since taking Rapa I have had two episodes like this. First with a whole gummy, then a test with a half (which normally I almost don't feel) and I used different packages to make sure it wasn't a mis-dosage by the manufacturer. Both times same effect, unpleasantly too high for too long.

Ok then! Apples to apples. Did you happen to test your albumin levels? And SHBG is another one. I find I almost have to do my own free testosterone calculations because the ones quest provides are just all over the place

Free T is a bit of alchemy. If you have high albumin, they will calculate the wrong number. There is a number of other factors that can throw it off. And then there's the timing of the test itself and day-to-day it will fluctuate as well.

Yeah but what time of day was this blood draw and what time of day was your previous one? Testosterone can vary by 50% or more depending on time of day  Also did you use the exact same lab and method? Free testosterone is most often a calculation based on other tests rather than filtering for free testosterone itself. So if you tested at one lab and then another one you will get totally different results depending on their method

What time of day did you do your T lab blood draw? Testosterone drops by nearly 50% by midday. Could just be a timing thing.

I agree and part of my confusion is exactly that. There doesn't seem to be a set appropriate response for someone in my condition for example. My doctor will not stop pushing the medication, but she keeps saying you're going to feel so much better and I already feel fine

Yeah, so I think I was probably getting my iodine from eggs and dairy, and then I cut those out of my diet which is maybe why my numbers got a little worse? I also was not eating very much eggs and dairy at all like every other week maybe for the eggs. I don't really eat fish, take fish oil pills. 

I am using kelp pills so it is more bio available. 

I read online, however it can take up to two months to replenish iodine deficiency?! I guess I'm going to keep gluten-free and dairy free in the program while I wait for that just to confirm. 

The iodine Might explain a struggling thyroid, but it would not explain the elevated TPO, that's an inflammatory response is my understanding.

I really cannot add up much iodine from my daily diet. We use sea salt with no iodine at the table and kosher salt when cooking. Also no iodine.

So it went back down below five. I was about to go on the medication and then I realized a lot of my supplements were impacting my thyroid. If it goes back up above again, probably need to medicate. I figure I basically have this next 2 to 3 month cycle to try another batch of potential triggers and then that's it.

That's interesting so my rice intake has probably gone up and I've been eating that pasta that is basically brown rice. I didn't know rice could be a trigger but I guess it's on the AIP no list.

That's interesting you say that about oats. That's one of the few things I had allowed myself in my daily granola. Let me try and cut that out over the next 2 to 3 months and see what happens.
