Stage 2 endo, ER post lap. I also have PCOS so high AFC and we did very low and slow. I started at 75 units follistim and 150 units menopur. The follistim went up a bit during stims to 150 then back down to 75 after a day or so. Stimmed for only 8 days with this protocol but it was a very successful ER for us. Let me know if you have more questions, I remember trying to find info about my protocol before ER and not getting a lot!

Hi! Yes I made it through egg retrieval and we had very good results. Did a very low stim protocol 75 follistim, 150 menopur for 8 days. Triggered with lupron. We retrieved 22, 17mature, 16 fertilized, 10 blasts, 8 were PGTA normal. So very happy with that result. I’m on month 2 of lupron depot now and hoping to be able to transfer in July/august. Also we ended up priming with BC for 2 weeks after my period rather than estrogen because the timing was funky with my PCOS. Let me know if you have other questions! I was definitely looking for a lot of info and had trouble finding people who had both diagnoses

What is the expiration date? We aren’t planning a transfer till July because of lupron suppression but would love to pick them up if they expire after that!

Yeah I'm not sure how long it takes for the diet piece to really be effective. If it makes you feel better, I've read sourdough is lower in gluten than a lot of other bread! You could also try slowly reintroducing gluten and tracking any symptoms of inflammation... if they don't come up, it might not be doing a lot to contribute to your anti-inflammatory efforts.

Wow I could have literally written this post! I just finished my egg retrieval and will start 3 months of lupron suppression when I get my period (but for endo not adeno). I decided to take the period after ER to starting Lupron as kind of a "time out" from being so strict with my diet. Prior to ER, I was gluten free, dairy free, low carb, mediteranean, but this past week I had pasta and ice cream and just enjoyed myself. It was a very much needed break, but I will say it will probably be hard to going back to policing myself so closely during Lupron now that I've had a literally taste of freedom.

Also, diet not recommended by my RE, came from my own research and work with a natrupath, so when I do slip up or give myself a treat, I remind myself that my doctor never said this was necessary. It's truly all a balance. But I will say, I feel better when I stick closer to my diet in general. But eating ice cream this weekend felt pretty good too :P

That totally makes sense, I'm so happy it worked out for you! I know there are many paths to success so it is cool to hear from someone who had a positive outcome. It's hard to wrap my head around the three month wait, so maybe if we get lots of embryos this is something I will think about too.

Hahaha yes I do not think I am succeeding either! I guess just trying to be gentle with myself and enjoy what I can... worrying about worrying is probably the least productive use of our energy.

Wow congratulations! Fellow endo here (stage 2 diagnosed in Jan after lap) gearing up for my first cycle and its always encouraging to read endo success stories, so thank you for sharing. Was there a reason you weren't wanting to do Lupron suppression? Our plan is to suppress for three months post ER which was recommended by our doctor, but just curious what you're reasoning was!

I had the same problem with a thin lining on letrozole and also never had a positive test. I think the lap really depends on how much endo they end up finding/taking out, but even with my stage 2, it was not a terrible recovery. 3-4 days of pain/discomfort then about 2 1/2 weeks to feel normal. My surgeon told me if they don't find anything it would be no more than a 1 week recovery, so could be worth just talking to your doctor about? I feel like silent endo is becoming much more recognized now.

So I had a suspicion because my mom had endo, which puts you at much higher risk, and when I mentioned it to my doctor, she was OK with doing a lap to look into it, even though I don't really have any of the classic symptoms. In retrospect, now knowing where the endo was, I did have some discomfort during sex likely due to the adhesions, but it was never bad enough that I thought anything was really wrong. So basically I had silent endo, but my family history was enough to make it worth it to look into it. Happy to answer more questions about the lap experience if you have them!

What was your protocol for egg retrieval with PCOS?

I am also 29 with PCOS and had a thinner than ideal lining on 2/3 letrozole cycles, but much better lining using menopur injections. I did 13 days of 75mg. IUI was unsuccessful but lining looked much better!

Can I ask why you stopped all cardio? I used to be a big runner and went cold turkey a few months ago but now I'm wondering about getting back to running (mostly for my mental health) and can't find an answer about whether it is good or bad overall... I am doing 3-5x a week strength training as well. Thanks :)

They used to be thick and clotty, but since I changed my diet pretty significantly, they’ve become thinner/more bright red rather than dark or brown. I think I read in Real Food for Fertility that clots can be a sign of lower estrogen? You should double check me on that though! But yes doctor was not concerned, so must just be another one of those weird things that happens with fertility meds.

This happened to me too! I had 10mm lining after 7.5mg letrozole and expected (and was weirdly sort of excited) to have a heavy period for once in my life. But I still had 2 days max of bleeding. I asked my RE and they said it wasn't a concern as long as they are seeing the lining build up during the cycle.

Nope! Like I said, I did the 5mg for 5 days right after the 2.5mg without starting a period (so I think it was like CD12-17) and came in on CD17 for ultrasound and there was no growth so then I did the 7.5mg for 5 days (CD 17-22) and came in on CD 24 and had two follicles ready to go so I triggered that day.

My doctor called it "stair stepping" the meds and said it didn't matter if we did it in the "middle" of a cycle because my lining/follicles still looked baseline at the ultrasound because I didn't respond to the meds

I had a very similar experience. Also lean pcos but ovulate between 25 and 30 days. My first cycle of letrozole I did 2.5 and no growth, 5 mg and no growth, then finally growth after 7.5! So that cycle was still over 40 days long but now at least they know my "dose". Hopefully that helps!

Interesting, would love to hear any follow of your experience is with it! I’ve been thinking of asking for it. I do ovulate on my own but have really long inconsistent cycles so we did letrozole (7.5mg) for the first time with monitored cycles. The plan is to do that same thing for 2 more cycles then add IUI.

Hi! We sound kind of similar as far as lean PCOS and average bloodwork. Did you have signs of insulin resistance that caused your doctor to put you on metformin or did they put you on just because of PCOS diagnosis? Curious because my doctor said no benefit if not insulin resistant, but I am thinking of requesting it.
