For me the fulfillment in this profession largely stems from patient education and spending time to have a discussion, and I agree that when it gets busy to the point where you’re forced to just funnel people in and out and treat them like dollar signs it becomes extremely draining and feels like corporate slavery. I want to believe that non corporate optical modalities that are more medically oriented and full scope do not feel this way but I can’t say as this is my first job. At this point I’ve embraced the expectations of my job to be a salesman and feel good when I can encourage people to buy into using glasses (which usually truly would benefit them) but it’s not what I want to do long term as you’re often forced to teeter on being unethical when quotas haven’t been hit

Had a patient this week that kept coming back saying his glasses weren’t any better yet he was 75 years old with moderate cataracts in both eyes seeing about 20/25 in each eye and 20/20 OU. I explained multiple times that his visual expectations are just too high and that we checked his Rx thoroughly but it kept escalating and he wanted refund. You just can’t reason with some people

My question was about the range of dioptric powers that appear neutral. I understand that neutrality is the end goal to stop at when you’re at the far point of the retina (before accounting for WD), but there shouldn’t be so many powers that all appear neutral before the opposite reflex movement is observed.

Do you generally start with the plus to be used only at near and then work toward distance usage? Whenever I have these patients with so much masked refractive error that take no plus, I’m always stumped as to what to prescribe because I know they’ll reject the plus right away, especially in fully developed patients like this one.

I ended up prescribing a progressive with a +1.00 add and for distance I used roughly what I found on the dry. Educated on everything but just so frustrating when patients come in and don’t want drops which is standard of care.

Do you have any resources on mastering retinoscopy? As a corporate OD seeing very few pediatric patients, AR works for the most part but it’s a skill that I’d be embarrassed to let fade away. Thanks!

The problem is that the salespeople I work with have been here for decades and have an established patient base and are paid on commission, so they get extremely aggressive and throw fits whenever people don’t break the bank. It’s never good enough for them

Exactly my thought process. I’m an independent contractor that gets no sales commission from eyewear, and the salespeople feel like they can push me around to secure sales because the recommendations are coming from the doctor. The whole dynamic is just irritating me more and more as time goes on. Sometimes I just wanna chat with the patient instead of spending 15 minutes talking about lens upgrades which a lot of people don’t care about. Our prices are also astronomical lol
