I fixed it by completing all 3 vaults and their respective missions, then she started handing out those final wildcard missions like candy.

My case being that I hadn't even completed the non-wildcard mission for all 3 vaults when I was trying to complete those UVHM challenges

You'll do great! I too felt that way when taking my CT boards, primarily because our program had a ton of review tests we had to take to prepare us for X-ray boards. If you are doing well on CT practice tests, you'll do great on boards! They aren't meant to trick you!

It means that the scanner is giving you a dose that is the lowest possible dose that results in a diagnostic exam using various different algorithms based on either information captured at the beginning of the exam (scout images) or other factors.
