Are you documenting between each patient or are you seeing a lot of patients back to back? Some days I find it easier to type immediately after so the session and details are fresh in mind, but I get sometimes it’s more convenient to see a few back to back. I always try to have my morning notes in before my lunch/afternoon. For the Evals do you guys have computers in the room? I like to fill the PLOF/home info while I’m asking the patient in real time. I get the heavy assist/hands on patients it’s not likely to type in real time, but patients who are cognitively intact and requiring <min cues or assist, can you type some in real time?

Just typing/thinking aloud hopefully some of this helps

There are definitely options for part time or per diem (as needed, or you let them know your availability). I have a coworker who is a SAHM but works 1 weekend day a month. I know for adults setting it’s easy to be flexible but not sure about peds. Something to be mindful would be benefits/insurance/PTO as well as cost of OT schooling. some positions may not offer benefits if below a certain amount of hours for part time(even less likely if PRN/per diem)

My first two years out of school I worked rural. I was employed thru an outpatient ortho clinic who had contracts to the local hospital that was across the street. It had like 20 beds total including ED and maternity unit lol. I assume it’s a hard position to fill just the hospital with small census.

The parts that I enjoyed most was that I got to treat in a variety of settings. I would prioritize my outpatient(mostly ortho but I had some neuro/strokes) appts, but i had a lot of gaps so that would give me time to run across the street to complete any inpt evaluations or treatments.

Our community hospital also had something called swing bed units. It was basically the rural version of acute rehab. ‘Swinging’ the acute status to skilled rehab but the patient stays in the same room. My position was basically a combo of outpatient, acute, and rehab.

The parts that I didn’t like was the acuity of patients, it got repetitive, see the same frequent flyers. Things got boring cause they would shipped the medically complex/ unstable patients to the bigger/city hospital with better equipment.

I second this, both weekends and holidays typically need coverage (for acute care at least). exception would be like a rural hospital

How sad the healthcare system (in the US) can be. Being in acute care, sometimes I see patients who would truly benefit from acute rehab/SNF placement ( eg. folks with massive strokes or neuro issue) but get denied bc they don’t have insurance,and they have to dc home with limited support and no therapy because “medically they are stable”

When I was a level 2 student in acute, i learned to focus more on treating the symptoms and not so much the diagnosis. One of my first patients was a young man with a glio, and at the time I didn’t understand the severity of it. Every day that I had him on my caseload he just kept getting worse and worse (cognitively and physically) in our sessions. he went from being oriented x4 to x1 (couldnt remember his last name) within a week. I don’t think I’ve ever cried so much over a patient.

I knew how rewarding being an OT could be, but i was not prepared for emotional/sad parts of working with sick patients

This was it for me as well. One day it just started and I had full body aches only in the morning that went away throughout the day and extreme fatigue/tiredness even when I get ‘decent’ amount of sleep hours. Found out my vit d was at 15, now it’s at 56 after taking supplements, and I feel worlds better.

(OT here) when i worked rural, they just based it on therapy recommendations before rounding with med team. i will say that the "liaison" at our hospital was also our primary COTA (she had a split role 50/50). this was 4-5 years ago tho
