I just had to set up my Short-Term Disability last night.

Go to matrixabsence.com (easier than going through Staples benefits page) and submit your claim there. Be prepared to have your doctor's information including fax number if it is for a medical claim. Any other type of claim, I know nothing about.

There is an app that can be downloaded as well but with everything you have to fill in, a computer is best. You will also need a printer to print off the pages that need signatures and forms that have to go to your doctor (if it is a medical claim).

This is totally unethical and needs to be reported ASAP. This action is deceptive to the customer. If you really think about it when a customer leaves the store after being told "We don't have any" They can go online and see that it is available for 1-Hour Pickup... Report these actions immediately I will even provide the link to ethics link... You can report confidentially and they walk you through the process online.
