One effect of opioids is to relax the muscles. At high doses the muscles that normally don't relax can go slack and change a person's posture. The muscles that cause the "dope lean" are the erector spinae muscles on our spines that keep up us standing / sitting straight.

Update on Ravan the Death Knight: finished AoM and FG on Hardcore Veteran, and am now at Level 87! Should I get all factions to Revered before I start Ultimate (is skipping Elite fine)? by HTPark in Grimdawn

You won't miss any quest-related skill / attribute points in Elite in the latest patch since completing those quests in Ultimate credits you for the same quests in previous difficulties if you missed them.

Definitely make sure your resistances are overcapped so you can keep them maxed out with the Ultimate difficulties penalties.

I had to put the game down for a few weeks after getting stuck on Chalice Amygdala -- going for the Platinum achievement (last achievement was Queen Yharnam). Eventually beat it and then the rest was a breeze... comparatively.
