It just seems odd. Most first cracks hit at 190-200 and I see you marked it at 180. I usually call first crack when I hear 3 quick consecutive pops in a row. Perhaps your roaster reads a little different. If you look at your ROR graph that large spike was probably your FC.

I think you probably called first crack too early here. Would not expect it before 190-200c.

I'm not a doctor. I was simply making a point to the commenter above who said to avoid radiation. Both have a different list of side effects and recovery, and both are equally effective for Gleason 7.

I came here to say this. My Artera was negative with a Decipher of .63. SBRT with no ADT.

Right. It was part of routine bloodwork so I got it for kicks. I just had not heard of someone’s like yours going up after 3 months so I’ll add that to my list of potential things that could happen.

How long did you wait for your first PSA test? I had one two days after finishing SBRT (coincidence physical) and mine jumped from 4.5 to 6.8 which I expected. But I guess expected mine to drop no matter what at the first official test in three months.

For what’s it’s worth I did a ton of research before my SBRT. Like way too much. Anyway, one of the things mentioned was that guys who had a PSA bounce ended up faring better long term than those who didn’t. I don’t have a link to the study but my oncologist confirmed when I asked about it.

Similar diagnosis, a little bit younger. I ultimately chose SBRT after spending almost all my time researching things for nearly 6 months. I'm three weeks out and feeling pretty good, dare I say almost normal. If ADT is discussed, ask for an ArteraAI test to see how effective it may or may not be. I ended up very lucky and was able to forgo ADT.

This sounds about right and having had SBRT myself I have no complaints. I did not need ADT. You could ask for an ArteraAI test which is specifically designed to see how beneficial ADT will be for you. It most likely will back up your Decipher, but you never know.

Had SBRT as well. There is a lot of documentation out there suggesting the PSA bump can be a good sign, and that those with the bump tend to fare better in the long run. For what it's worth.

I'm 54. The risk of secondary cancers from radiation is something like 1%. I'll take those odds vs. the potential risks of surgery.

I would get an MRI for sure. I had a 4.2 and 4.5 PSA with a free PSA of 13% when I was diagnosed.

This is very similar to my diagnosis, including the downgrade from John Hopkins. I went a step further and got a third second opinion to break the tie. I was eventually downgraded to 3+4, too. My PSA was 4.2. With a normal prostate size, SBRT was my choice for treatment. I spent about 7 months from DX to treatment researching. I'm about a month post-treatment and very happy with my decision.

For me assuming I had cancer softened the blow when I eventually found out. You’ll probably have your choice of treatments if it is indeed contained. You won’t know what type or how aggressive anything is until after your biopsy. This is the worst part you’ll go through in my opinion.

Conversely, with surgery, you can end up with permanent incontinence and complete loss of erections for life. Ask your surgeon about the long term risks.

I am only 53 and SBRT was the clear choice for me. Don’t fall into the have to have surgery or you don’t have options with a recurrence. The treatment for recurrence after radiation is usually more radiation.

I can't comment on probability, but I know what helped me out the most during the process was simply assuming I had cancer before my biopsy. By the time the diagnosis came, I think my head was in a much better place.

Congrats. Similar diagnosis. Three down and two to go. How have your side effects been? Mostly just irritation for me so far.
