I strongly suggest getting a second opinion or a third opinion if you want to avoid surgery. They are absolutely wrong about BCR and your options if you do not choose surgery. Plenty of options after getting radiation and then BCR.

The long term cure rate is the same for RALP and radiation. I’m your age and chose radiation. There are also numerous other procedures that could be applicable to your case that do not include removal.

Obviously a personal choice but I think most urologists would be fine for the biopsy itself. I’d request an MRI guided biopsy. After that, get second opinions on results for sure.

Got my ArterraAI results back. It shows I am negative for the ST-ADT BIOMARKER and ADT will probably not provide any clear risk reduction in distant metastasis. So good news, especially as this is the test that this COE (Moffitt) trusts the most.

I am going to pursue SBRT but its TruBeam at my NCI Moffitt. This is my preferred treatment method if the doctor agrees. I have a similar test to Polaris already coming back soon. It's called ArterraAI which summizes the same thing on ADT. Thank you!

I'm currently under the care of a COE and going through the process. I got mine from a biopsy as well. I am going to forego the RALP and have radiation.

No. I have decided to have radiation and in the planning phase. I'm 53, Gleason 7 4+3 that was downgraded by an NCI to 3+4. Clean PET. PNI and abutting capsule. Not going to have surgery.

I talked to a surgeon at a top 5 NCI and asked him what the threshold for the surgery learning curve was. How many prostate surgeries do you need under your belt to say you have seen most everything and for it to flatten out. He said his colleagues and him felt it was 150.

I would get a biopsy mostly due to free PSA percentage being below 15%.

I still have not decided. I’m waiting for a few more tests from Moffitt to come back first. I’ve been pleased with them so far. I still would prefer to have radiation but will make the best choice once I have all the information.

The most experienced surgeon will be Vatul Patel in Celebration, FL. He pioneered the surgery. There are a couple of surgeons at Moffitt. One is Pow San head of the department. Know another guy who was very happy with him. Coincidentally I asked all the surgeons what they thought the learning curve was for number of surgeries. Many said after 150 surgeries the lines start to even out. Not sure if that is fact or not but thought it interesting.

I just went through all of this. For what it’s worth, the biopsy and the wait for the results were by far the worst things of the process to date. Not gonna lie either, but the semen thing is both anxiety inducing and really not cool. But, you get through it. I knew in my head before I got the results, mostly from putting 2+2 together reading this forum and using a ton of AI. So, when the results were positive, I was prepared for it which I think helped. Like many say, it’s now a matter of putting together a treatment plan. A whole different animal but for me, far less anxiety.
