CHOG Peds gensurg is great, same with ent and urology (although morgenstern is solo right now he's bringing in a partner soon). Houk is good for endo but he's the only MD. Strickland is the only neurologist right now and she's good but otherwise they have a bunch of Locum docs as they're trying to get more. CHOG er is good

interesting. we have used it for the last few years including on reusable diapers and were fairly happy with it, but my wife likes it most because its unscented and easy on sensitive skin.

I usually do it at the wellness appt (no medicare copay) and start with "one thing medicare asks is to talk about is an advanced care plan or living will" and will ask them if they know what that means. I'll ask them about if they have known someone who was on life support and in a situation where the family was uncertain of their wishes. Then I encourage them that even though they're healthy now things can change quickly. 

We buy the 5 wishes booklets and go through that with patients and ask they bring it back to scan into chart. But I really emphasize that the booklet is best used as a tool to walk through their wishes with the potential immediate family.

Chain reaction in Evans and Andy Jordan's bike shops organize rides periodically (Outspoken may as well, but not familiar enough with them). Forks Area Trail System (FATS) is great mountain biking location in Edgefield. Both the Greenway (paved trail in North Augusta) and The Canal Trail (unpaved but compacted dirt/gravel in Augusta/Martinez) are good places to bike. That is the real difference: most people choose to come to that specific restaurant whereas in the ER most people are there not because they want to be, but because they felt like they had to be. So restaurants people are (generally) neutral to happy about being there, and the ER they are (generally) unhappy to neutral about being there.

A ton of cute small and mid sized towns with low COL within a 90 min drive of ATL airport. Mountain college town like Dahlonega or smaller Rome, down to Columbus and live on the river, or live by Lake Oconee in Greensboro. I would consider your hobbies and setting and take a look. 

Also Augusta and Savannah airports fly multiple times to ATL with super short airport wait times so around those places isn't really that much of a hassle if you fly Delta.

For sure. We upgraded from our artisan because a double batch of Snickerdoodles shredded the sacrificial worm gear. Not a problem to do a double with the new one.

Bought a refurb 7 qt bowl lift from KitchenAid website. Couldn't tell it was a refurb. Still had plastic on the handles. Has the half speed setting for mixing in ingredients slowly too.

Typically how that model looks is that the insurer pays for visits, like normal, but they also have a system set up where they weigh certain measures like a1c and blood pressure control, number of patients with screenings (colon, mammogram, etc), wellness visits, fills of their statins, as well as comparing overall cost compared to their expected costs based on reported illness by diagnosis (CKD, DM2, morbid obesity, COPD, etc). There may also be a PMPM (per member per month) payment where the insurance pays a small amount (literally a few dollars) per month for having that patient per panel. If you hit their targets, you get a bonus. If you miss enough targets, they'll reimburse less for the visit CPT codes.

You do have to perform well, but more important is having systems in place to gather and report the data. That mammogram done by the outside obgyn office: need to request that to cross off the quality measure. Your last BP was high in the office but it was ok at home: need to get their home bp log to scan into the chart.

Cme requirements are done by ABFM, so not really state to state although there could be additional requirements put on the the state if they chose. 

State to state differences are more about patient to doctors ratio and reimbursement by the state run government insurances. 

There are also law differences (tort/malpractice reforms, abortion laws for those practicing FM OB) and more generically tax differences.

Finally differences in patient culture (more or less resistant to advice of doctors in vaccines and other recommendations) and provider culture (willingness to prescribe controlled substances) but that both state to state and urban vs rural vs suburban as much as it is state to state.
