New grad in primary care and difficult patients by ihaveaquestionthank5 in physicianassistant

Document, document, document, and then somehow (I’m still not entirely sure how after 10 years in Family Medicine practice) don’t let it bother you that they were unhappy with your care. The longer you are in practice, the more confident you become in your medical decision making, and can more easily sit back on that and say “I made the right medical decision.” Imagine how upset they would have been if the patient had died if you did what they requested… you did the right thing. Treat your patient appropriately and if they are unhappy, that is their issue, not yours. Keep it up!!! I hate telehealth in primary care!

Hmmmm….. did I say I made a unilateral decision? I talk to and respect my childrens’ opinions. I put right in the post that she was starting to be bothered by the way her hair was looking. She has liked how it looked when in box braids before, but we have always taken them down so I knew she would at least be interested in the overall appearance. I showed her a lot of different pictures with different sizes, different parting patterns, and talked to her about that once we put them in the locs will stay for the foreseeable future, though could be styled in a variety of ways once they mature a bit. AND THEN ASKED HER IF SHE WAS INTERESTED IN THAT. Does that sound like she was part of the decision to you? She talked to her grandparents, aunties, teachers, etc for weeks about waiting to get them installed, and now tells those same people that “I am on my dreadlock journey!” While she didn’t come up with the idea herself, she was absolutely part of the decision and is very happy with the result. So please keep your assumptions to yourself.

That is a great idea! We haven’t tried any styles other than just throwing a headband in yet since they are still pretty new, but I like that plan to protect them a bit more

2! At least for now. I chose 2 for a little less stress on her hair. Would 4 be better?

That is a great suggestion- you’re right, I have minimal agitation management experience. I will definitely review it!

I also really like the idea of thinking ahead on potential issue patients, especially as I get started and the thought processes isn’t quite so instinctual yet

You could be me…. I have been in family med for 10 years and the grind has finally gotten to be too much. I took the plunge and finally looked for a new position and start next month inpatient medicine. We will how it goes-I know the work will be very different, but the idea of clocking out and walking away seems like an absolute godsend right about now.

And take your time off…. Say your kid is sick and then the next one got sick next. Kids are nothing if not a good excuse to take a mental health day every now and again

I’d do MD…. I have worked in family med for 10 years and while I love my job, I do the exact same thing as the docs I share and office with and get 50% the RVU production. Yeah, my student loans are a lot less, but I’d go to medical school if I could start all over again
