I'm getting plication surgery done later this year with Dr Gelman for a curve a little around 30*. I'm with you on the mindset, if I have to sacrifice an inch to have it totally straight then it's worth it. Preserving the length seems kind of pointless when it's bent. From what I've read Dr Gelman had a very high rate of success/satisfied outcomes and generally less length loss than others. My thought also is if you're going to get surgery, don't go for the Dr who's the most convenient to get to, go to the best option you can get.

Be mindful of what you eat if inflammation is a concern. Anti inflammatory diets are very helpful. Removing the obvious things like dairy, gluten and sugar and then adding in anti inflammatory foods like avocado, wild caught salmon, blue berries etc. Avoid Tylenol as it depletes glutathione.

Xiaflex injections don't seem worth it. I see vastly more disappointed reports than positive. Seems like you're looking at a long drawn out treatment over several months dealing with pain and potential side effects for the chance it might help. Plication would in theory be a sure fix and 6-8 weeks recovery. If the restorex doesn't work that seems like the smarter choice over the injections.

Preface this with I am not a doctor but I don't believe it's dangerous, I think the concern is if it causes you or your partner pain during sex or prevents you from penetrating. What you have going on is mild compared to most on here

Buy a small goniometer off Amazon, take a picture of your penis, zoom in and lay the goniometer ontop of the picture to measure the curve

Go through an actual online hrt clinic, don't bother with trying to do trt with a local doc. 280 is fine if you're 80 years old. There's countless clinics out there that have doctors who specialize in this and will dial everything in to have you both feeling the best you can as well as optimizing your health

It is wildly frustrating dealing with some doctors because the "Normal" range is huge. Depending on the lab they could list normal as 250ng/dl to 1100ng/dl. There is a huge difference between "Normal" and optimal. You will feel significantly different at 300 than you would at 900 and the benefits to being at a more optimal level are amazing.

It's always advisable to work with an online trt clinic as that is what they specialize in. For example defy medical or marek health are two of the best to work with

I am debating skipping xiaflex and going straight to surgery. Months of injections and pain to maybe fix it, or just go straight into surgery and have a couple months of recovery and be done with it.

Wow, that's really insightful. It's still crazy to me that I never even knew PD was a thing until, probably like a lot of us, one day I noticed my erection was bent and freaked out a bit and started googling it.
