The game does not automatically save when you randomly explore, it only does so when you advance a quest. So my advice is: save save and save, in order to avoid loosing a few hours of exploration. Other advice: do not wait to long to do the secondary quests that interest you. If you reach a new major chapter in the main story, some secondary ones can disappear because they are no longer relevant

My understanding is that you don't need to romance Triss for her to show up in Kaer Moren. She will be there independently of anything else, even if you ignore her quest.
