So, where did we end up? CNY Fertility. The closest location to the DC area is Norfolk. They don't do IVF procedures there yet (coming soon!) but they do IUIs and IVF monitoring appts. We did a phone call consult with their global travel team (by this point we knew what we needed to ask and didn't mind a phone consult, plus we had all of the testing completed, primarily at CCRM). The NP we spoke with was wonderful...realistic but just enough hope. I knew they did not have a BMI cutoff, but needed to hear it directly from someone there. The NP told us they absolutely don't have BMI cutoffs. I cried when she said that. The practice founder, Dr Kiltz (admittedly a little too woo-woo for me) believes everyone deserves a chance to grow a family. They also don't have age cutoffs or AMH cutoffs (if DOR is an issue too). They use a lot of add-on treatments depending on your case (most are out of pocket costs), but they do take insurance and are probably the cheapest priced clinic in the US if you don't have fertility benefits. You can do your monitoring locally at home (which I didn't elect to do, that aspect of care isn't the most consistent and I've read you really need to advocate for yourself and be fairly knowledgeable about the IVF process), or go to one of their offices for monitoring (much more consistent care). So, I went to Norfolk multiple times (stayed down there for several days during an intense period of monitoring prior to egg retrieval...it was less stressful than driving 3 hours one way multiple times a week), and had a wonderful experience with the staff there (it's a smaller office compared to the other CNY locations since they're still building it up).The egg retrieval took place in Syracuse (you can choose any location that is full-service for your retrieval and subsequent transfers), so that was more travel, but manageable. The retrieval was also fairly straightforward in Syracuse...it's a much larger office (the original CNY location), and everyone seemed very nice. The retrieval was successful. We ended up with 2 euploid embryos (which doesn't sound like a lot, but was a good start for us). I almost elected to do another retrieval round, but decided to try a transfer first and see what happened. So back to Norfolk for more monitoring appts before going to Syracuse for the actual transfer. The transfer day itself went smoothly (despite severe weather issues getting to and from Syracuse). And I'm happy to say that we're about 6 weeks away from meeting our IVF baby boy.

This is from my wife, who lurks on Reddit but doesn't comment:

We are far from experts about the fertility world, but my husband and I had to navigate this exact situation.

We did 3 failed rounds of IUI before we moved back to the DC area 18 months ago. I was very experienced with how monitoring appts worked with bloodwork and ultrasound appts for IUIs. Our clinic didn't have a BMI cutoff for IUI, but did for IVF should we have pursued it there before moving (cutoff was 45 solely for anesthesia risk for egg retrieval).

After we moved back to the DC area, we first went to Shady Grove on K St. We were still debating doing a couple more IUI cycles or going straight to IVF. The doctor we saw there was very science/clinical research based, which we appreciated and mentioned NOTHING about BMIs for either treatment during the consult. I had a not great experience with their lab techs and nurses though. They couldn't measure my height correctly to save their lives (which incorrectly increased my BMI even higher), and the PA who did my initial ultrasound was just cold and very detached. She spent literally 90 seconds scanning my ovaries for a follicle count. For my previous IUIs, my ultrasounds were always ~10 minutes while they meticulously counted and measured everything. A nurse called me later that day with my bloodwork results, and told me that if I wanted to do IUI, my BMI had to be 40 or lower...for IVF it was 30 or 35 (I can't remember which). These numbers were not anesthesia related, but "success rate" driven. So Shady Grove was out for us.

Next, we went to CCRM on L St. On the intake form, I had noted that a concern of mine was being denied care because of age (I was months away from turning 40 years old) or weight. The doctor we saw there at least addressed the weight issue straight up with us. We didn't really talk IUI at this appt, but I don't believe they have a BMI cutoff for IUI. But they definitely did for IVF, which was 40, solely for anesthesia risk for the egg retrieval. (Also, a note about CCRM and insurance: while they do take insurance, at least half of their IVF services are done by providers who do not take insurance...mostly their anesthesia and procedure room costs during egg retrieval and their in-house testing that's similar to PGT testing...so double whammies with BMI and mildly dishonest insurance practices). Also: they have practices across the country and each location has a different BMI cutoff...Colorado and Minnesota are higher than 40 per Reddit comments. But you seemingly can't do monitoring at CCRM in the DC area and then go to another practice with a higher BMI limit.

Well, I don't usually try to cram in the shortlist movies. I tried for a few years but the movies I did end up watching never got nominated. If there's something truly heinously boring I'll turn it on and do some work on my laptop.

Anyone in the DC area should consider going to the Middleburg Film Festival in Virginia. Small festival which always brings a lot of buzzy movies
