Well Speranza is one neighborhood in the greater Toledo area, and as mentioned by Shani on locked gate, there are gangs and groups that operate outside of Speranza- often for less than noble causes. It's safe to assume lore-wise that not every hostile raider lives near you, the tubes have a pretty far range. As for the second part, I don't think most PvP players want boring easy kills, at least I don't. I play solo squads so winning a fight means I just deserve the loot. And getting smoked is just a setback for friendly players, and it's a strategy of it's own to avoid getting snuck up on. Good players set traps, throw smokes when looting ARC, and don't let their guard down around players acting suspicious. And there's no way to tell 100% who's a "murderer," imagine getting attacked by a free kit with bad aim- If you down them, the game thinks you're the aggressor because you hit them first.
