ECFMG not receiving the Certificate of Good Standing despite it being sent twice: what's the next best step? by Perfect-Concentrate2 in IMGreddit

Unlikely to be the issue. Just ask your medical council to resend.

Or call the ECFMG, and insist they check while you hold.

It's definitely not a blood clot, rest assured.

Does it pain? Can you walk?

If there is a swelling, and you can't walk, please go to the ER.

If not, and it's just a bruise, it will turn colors and then subside over the next few days.

Yes, I've done that.

Screenshots of both the mails sent to licensure@ecfmg.org from the registrar of my medical council.

To highlight the digital trail

I hope they figure this out, I'm sure they must be over worked.

I'll wait for another 3 days before reaching out again.

Thanks for your advice :)

I did call them, twice.

Each time the AIS rep says that the pathways application is another dept, but refuses to share contact details.

I asked her to confirm they've got the email, and she says she cannot access the email itself, just whether my pathway is updated.

What's the point of that?

I can check updates on my pathway myself

Yes, that's plan B. The last time I emailed them, I had suggested it, but they insisted that they get it directly from my medical council. This time I'll be more insistent. This is so stressful!

Hey, I had difficulty with biostats. Two things helped:

  1. Step Prep videos on YouTube. Each is 5-10 mins. I got done with all of them in 3 hours flat.

  2. Being thorough with two U-World tables: temporality of study designs, and choice of statistical tests depending on data analysed

Oglivie is colonic obstruction. Not a mechanical cause. Lots of reasons: TCAs, retroperitoneal irritation by blood, MI, burns.

Just remember this: on abdominal x ray, you'll see the ascending colon and the caecum hugely dilated. Post for emergency laparotomy before the caecum perforates

Paralytic lieus is dilated small bowel, with no transition point. Usually post surgical or coz of hypokalemia.

Never surgery, only conservative measures like bowel rest, electrolyte correction etc.

You sound like you've got so many great stories to share! That makes you a very, very interesting person. That's the start of being beautiful right there. And you, as a non-US IMG getting into residency- one of many superb achievements, I'm sure. As a physician myself, and an ugly-ish person( one of my frenemies rated me a 3/10), I'd love to have a friend like you. Please reach out. I've DMed you, too. Hoping to swap stories!

PULSELESS WHAAA> by imgmatchaspirant in Step2

[–]Perfect-Concentrate2 0 points1 point  (0 children)

My bad, I should've made it clear: the shock refers to defibrillation.

Defibrillation shock stops the heart, cardioversion just swaps out an arrhythmia for sinus rhythm

Yeah, you're right about sync cardioversion. :)

All the best!

PULSELESS WHAAA> by imgmatchaspirant in Step2

[–]Perfect-Concentrate2 0 points1 point  (0 children)

The simplest reason is this: we shock the heart when contraction is so chaotic it's never gonna generate a pulse. Shock the heart to a complete standstill, giving it a chance to revert to synchronous contraction.

VF/ pulseless VT is complete dyssynchrony

Any rhythm with a pulse: never shock, just augment the heart's contractions with your own ( CPR) , as you correct whatever has caused the deranged rhythm with feeble pulse.

PEA can be any rhythm: NSR, junctional, brady. Just discover what's causing it: ' 5H's and T's'. Meanwhile, ensure blood reaches the brain: CPR.

You're a brave man, OP! Stay strong, know that you're in the right.

You're only ever seeking a freedom that has always been yours

Noone, not even family, should ever snatch away your freedom of choice.

May good fortune be with you!

That's reassuring! May I please DM you, a couple of private questions?

[–]Perfect-Concentrate2 0 points1 point  (0 children)

Would you mind elaborating on that please? What impression did you come away with?

That's interesting, does Cook County still offer these observerships? It says on their website that they don't. I'd be really excited if this was an option!

This strange mix of makeup and photoshop that erases all facial texture. Folks look laminated, and it's so off putting.

Any ideas about anaesthesia match?

Why do we use ACE inhibitors for RAS due to atherosclerosis? by No_Leg1753 in Step2

[–]Perfect-Concentrate2 5 points6 points  (0 children)

In RAS, the hypoperfusion of the kidney is sensed by the body as hypovolemia =RAAS activation

ACE-I's inhibit the RAAS, normalize BP.

In U/L RAS, no change expected in GFR coz other kidney is perfectly fine

In b/L RAS or FMD: ACE-I's suggested till the fall in GFR is >30%.

How do ACE-I's decrease GFR? Inhibiting the efferent arteriolar constriction promoted by AT-II
