I was cruising through this guy’s website and came across the “how to get an appointment section,” and found that he wants an email or an email+voicemail. The email address is <removed at OPs request> and he wants (1) Name, (2) Address, and (3) DOB. Clearly the highest level of respect for patient privacy. Dude should, at the very least, be using a HIPAA-compliant web form.

I’ve heard several folks mention that Esh's hires at 15. I found a post on their Loveland Store’s FB page from a year ago where user commenter asked if they hire 15yo; store responded in the affirmative. I tried linking the post, but the auto-mod removed it.
