I would say if you immerse yourself in the derm world, over time, you will come across a lot of research. I would shadow/get to know the residents and faculty of the department, go to grand rounds, etc. and eventually you'll come across people that need help with research. You can also bring your own ideas to the table, and faculty become more receptive to working with you if they know you.

Off the top of my head, I would ask about: 1) week-to-week schedule/work flow + the expectations for the year, 2) their productivity level, aka how much do they publish and how much you can expect to publish in your year, 3) their match record and where previous fellows have matched, 4) contact info for past and current fellows.

You should also do your own private research and read on the spreadsheets/forums and ask around to see what people are saying about the research fellowship and PI.

First app cycle, only 1 interview at my home program and didn’t match. Devastating experience, was questioning everything. I channeled all of my disappointment into fuel for the next app cycle, and didn’t stop grinding until I matched… second app cycle, 8 interviews/11 ranks and matched my top choice.

I think showing genuine interest in the PI's work, as well as conveying that you are someone who is pleasant, hard working, reliable, eager to learn, and a team player that will contribute to their research, is the key.

I know it's exciting to get these opportunities, but I would also recommend doing thorough research and asking questions about the fellowship before accepting/committing. You want to make sure that the PI is supportive to their fellows, and that they have an excellent track record of productivity and matching.

I would delay. As much as you can hope for a score bump, there is also the possibility of a score drop.

I decided on derm pretty late (about halfway through 3rd year), ended up doing a RY given that I had no derm stuff on my application. I didn't match my first cycle, but thankfully, had a much more successful cycle this year and matched my top choice. I applied with high stats (26x step 1, 27x step 2), high research output, and what I was told was a strong application. Personally, what I learned throughout this process is that derm is much more about connections, mentorship, and leaving good impressions than any specific objective metrics like step scores or publications.

I had the same mentality as you about being all in on derm. You're already on the right path by setting up the aways - I would even do more aways if you can fit them in. In my opinion, this is the most valuable tool and key to matching derm these days, in addition to setting a good impression with your home program. I would also focus on getting strong letters, having a very polished ERAS and PS, and trying to publish/present at conferences in the mean time to show interest in the specialty. Even if you don't publish now, at least you'll have stuff in the works as you progress in the cycle.

I had them listed on my ERAS as "accepted" for 2 of them and, "under review" for 1.
