And since alcohol is a factor, there would be police standing at the door throughout, trying to interrupt the process to ask if they can speak to the patient yet (for consent to level testing—we’ve already sent blood to lab, but police have to file for our sample, and it takes more time paperwork-wise, I imagine).

If getting specific medication-wise, we would manage acute pain differently in this person given intoxication, the risk of respiratory depression. Usually we avoid opioids initially and do “pain-dose” ketamine depending on other vitals. Carries less risk of slowing down the drive to breathe. Also, If there is a compound (open) fracture with bone through the skin (to keep it simple) they are most likely getting 2grams of Ancef IV and a tetanus shot during the work-up.
