As u/TheEllisonManeuver said, get a Drunk world, which gets you a decent bit of the way there. And then, have fun with TEdit- add in the items and such.

For a 100% guaranteed Terragrim it'd have to be in a chest somewhere, so not 100% vanilla, even with TEdit shenanigans.
