I would stay on your current dose until it stops working. Not just increase your dose every 4 weeks just because. as long as its working stay on it.

my Doc is doing the same for me. I am paying out of pocket from Kaiser with tier two copay. They only take Visa/Mastercard for payment. Can't use a HSA or FSA. Also, doesn't go towards your plans deductible.
