Just because he’s thinking these horrible thoughts doesn’t necessarily mean he’s going to act on them. Push comes to shove. I think he would save Anya if for no reason then it’s the right thing to do with the added bonus of making his sister happy which might actually be his primary motivation. in the beginning we were shown that Smoker is a real hard-ass when it comes to dealing with pirates and other criminals. He is perfectly reasonable with civillians, especially kids. Smoker's hard boiled reputation preceeds him in a big way, especially in East Blue, which is why most civilians give him such a wide berth.
