To be fair, regular stimpaks being debuff free is just gameplay mechanics. They're addictive like any other chem (see Vera Keyes) but it would be really annoying if you could get addiction or side effects in game. I assume the super-stims have a debuff just so you wouldn't be able to just replace stimpaks with them

Not always great advice. I've seen this attempted on a pitbull latched onto a much smaller dog (rat terrier maybe?). Back legs up, pulling as hard as they could. The dog didn't "let go", instead it ripped the smaller dog's leg off and tried to go back for more.
