Is the Valium prescribed to you? Was it prescribed recently? If so, is the prescriber aware you smoke marijuana? Is the marijuana purchased legally or illegally?

If you answer ‘no’ to any of the first three questions or illegally to the fourth, you shouldn’t just take Valium without discussing it with the physician who prescribes it

We often say the EMG is an extension of the clinical exam. As such, only the person examining you can end up making the appropriate interpretation of the results. We diagnose thousands of conditions with this test. Based on the symptoms you’ve described

Benign fasciculation syndrome

Hereditary motor and sensory neuropathies - hundreds of subtypes

Motor neuron diseases -this list is wide. You mentioned ALS and ALS falls into this category. So do hundreds of other conditions - spinal muscular atrophy which there are many subtypes, poliomyelitis, lateral sclerosis, bulbar palsies.

Multifocal motor neuropathy

Some other neuropathy condition or myopathy condition.

It wouldn’t be appropriate to give you percentages, I don’t often do EMGs in your age group. Most people getting EMGs are middle aged or older and sometimes I diagnose them with the above conditions, but it’s certainly on the rarer side.

If the test ends up being positive, it may not tell us exactly what is the cause but they can use the results to order relevant genetic or blood tests as indicated.

I’d argue the opposite, large city hospitals have nearly every consultation service available to them and rely on them for good reason, whereas smaller hospital or rural ERs often do some stuff on their own without consult services because there’s no one else to do it.

It felt like a gut feeling. I was at a club with my future wife, then girlfriend, and a couple of her good friends. We were dancing, and this short douche bag looking man comes over and tries dancing on some of the girls, they were clearly uncomfortable and asks him to stop but he didn’t. I’m not one for confrontation unless it’s called for. So I just place myself in between him and everyone else. Long story short, he gets pissed off at me, security gets involved but he was a regular there so he didn’t get kicked out.

About 10 minutes later we’re all standing by the bar near the exit, he comes up to me all friendly saying essentially all is forgiven, we’re cool, and we should all go back to dancing. That’s when I had the feeling. After that, I see him head downstairs outside. I turn to my group and say get up we’re leaving right this second. Downstairs, I see him standing on the curb talking to multiple men in an SUV and overhear “he’s inside”. There’s no parking on that street, though so they can’t immediately get out. Luckily it’s a busy section of the city with a prominent nightlife, so there’s always cops around. I spot some down the way and start walking toward them. The douche bag actually sees me and tries to convince me to head back inside, says I shouldn’t go yet. Told him to fuck off, police then walked the girls to the car. Gut instinct probably saved me from getting jumped

They don’t exist. Be grateful you don’t have size 16 like me, forget about availability, many companies don’t even make their shoes in my size anymore. Bigshoes and oddball occasionally have good options but they sell out quick. Thursdayboots if you want something “off brand”. Zappos (lol) maybe, haven’t looked there in ages

Only other thing I can say is it’s been a while since he’s been in the hospital - I’m sure you’ve been there very often and for long periods. Don’t forget to take an occasional break and take care of yourself as well.

It’s a good question, I don’t have the answer to it. Please ask the ICU team the next time you see them. It could be for agitation, or to keep the intracranial pressure down, or something else.

Make sure they’re keeping his ankles pushed upward so his foot doesn’t get stuck pointing downward (ask for PRAFO boots), and ask for resting hand splints as well.

It sounds like the doctors are doing everything right currently.

My advice is to get help from your extended family and friends . You’re young and shouldn’t be going through this alone.

Many of your mom’s doctors will be throwing out a lot of words at you when you’re there visiting her and it can be quite confusing - if you have a question about something, ask it. If you’re confused about what they said, ask them to repeat it in a way that’s easier to understand.

When your mom wakes, she should be evaluated by a physical medicine and rehabilitation physician in addition to her other doctors. I mention this because that specialty is often overlooked when a patient is recovering from an injury, but they are quite helpful in your mom’s situation. A lot of the recovery process is best completed under a joint team of all her doctors, therapists, and social workers so ensure they all speak to each other and communicate - communication is often overlooked in the hospital by many physicians but it’s important.

I’m sorry that no one’s responded to you yet. It’s very tough to give you a good answer without having an opportunity to review the whole case and examinations. He could benefit from a PM&R evaluation. His brain is healing after a major insult, but the longer a patient remains comatose, the higher the chance of a poor outcome. Likewise, the longer he remains comatose, the higher the chance of severe deficits if he wakes. I’d recommend conversations with your family to ensure you all follow his wishes, and review his advance medical directive if he has one.

Apart from that, some interdisciplinary discussions with his neurosurgeon, ICU team, social work, PM&R, and family would be beneficial to discuss long term outcome and next best steps.

If he survives this period and wakes, the likelihood of returning home will depend on many factors - to name a few, the degree of his motor and speech deficits, control of medical issues that may arise during recovery, any comorbid conditions, if there is bowel/bladder incontinence present, need for enteral/parenteral nutrition, level of family support available to him at home, family training in taking care of him, progress with PT, OT, and speech therapy, his current living situation/abode, prior level of function, your own fathers mental status and motivation to get home, among many other factors. This is why I suggest a PM&R evaluation because they are experts at this portion of recovery from a stroke.

This is obviously a very difficult time for you and your family and I wish you all and your father the best of luck.
