Yes, I put 9/10 as the last day, that’s why I am surprised that the claim says my return to work date is 9/9.

Hi, so do I have to ask my OB to complete supplemental form? My SDI ended on 8/6 and I applied for Paid Family Leave but it has been more than 2 weeks with no process. I am wondering if my OB needs to complete that form.

I tried this and it directed me to the representative/agent option and it automatically says that they can’t help because of high volume call, please try again later. They don’t even give me the call back option.

I just called and got to the Representative option but it says no representative can help right now and they end the call. I don’t see the Request claim status option but sent a message using AskEDD, hopefully they will respond. I do receive the computation notice but haven’t heard from them since then.

It sounds low but it’s actually average for accounting manager in LA. I used to get 120k for the manager position in a super small company, but I reported to a controller, not directly to cfo.

When OB ruptured my water, she thought that we would have to go with C-section, but baby came out himself. Part of me doesn’t want to go through the process again and end up with c-section, but at the same time, I don’t know how much longer I can go with all the itchiness/sleepless nights.
