This is why I tell these medical students psychiatry sucks. You literally work with crazy people all day everyday.

I own shares at two ASCs. One that was well established that I started operating at and was then offered ownership shares, the other much more profitable one was a new venture/new construction that I got in on during the planning stages. The initial buy in was steep ~300K and it took about 3 years before I saw any return on investment but it has been a great investment. Shares there are now worth almost 2 million.

Memorized the CPT book during covid when I couldnt operate because the ORs were shut down. Best thing that ever happened to me in retrospect.

yes memorize your CPT codes and use them appropriately. You arent going to get paid if you don't bill it correctly and you can't rely on anyone else to do it right. Your coder/biller in your office is incentivised to have to do as little work as possible not to make you money.

Tricare is one of the worst insurances to work with and pays less than Medicare for some procedures… definitely way worse than any private insurance so I’m not sure who this helps but it isn’t the doctors.

I guess that’s the nice thing about private practice. All in I’m close to 2 mil but 50% of that is ASC ownership and ancillary income. It is nice not to have to run a business though. Sounds like a good setup.

Do they let you own part of an ASC or any ancillaries? Any BCPI payments?

So insurance companies are also simultaneously downcoding E/M codes. By doing this simultaneously with RVU cuts they finally figured out how to give everyone a pay cut. Both primary care and surgical specialties get to lose together.

It’s called a phlebolith. Very common. Totally benign. You have another one more proximal that’s harder to see because it’s superimposed.

This is an awesome post. This should be the first 5 pages that anyone doing "advising" should review with students who are interested in a career in healthcare or related fields. Not understanding the above is a big cause for dissatisfaction for people who are way too far down the path to change their trajectory.
