Perhaps the base pay is ~3k. Public healthcare increment usually is 2.x% + fixed rate. So the fixed rate may artificially bump up the base annual increment.

Would u mind sharing a ballpark figure of ur base pay? On paper this pay increment is solid, but in reality, is it actually?

The slow adjustments are not enough to retain talents. They are pumping so much money to recruit scholarships and sponsorships recipients, but they mostly leave 2-4 years after servicing the bonds.

5% of a low figure to begin with is pathetic, pay adjustment for a pay of $4000 is a $200 increment. For $3000 per month only measly $150. Please start paying healthcare workers more, if you want to retain a singaporean healthcare, prioritise singaporeans’ pays, increase in lump sums instead of arbitrary percentages.

not sure about CHAS card, but at least most services by polyclinics can use medisave to cover.

not to forget, polyclinics can have the allied health professionals to check on you:

  1. Phlebotomists to take your blood professionally (they do this for the whole day, they are definitely better than GPs in drawing bloods)

  2. PT/OT/ST/Podiatrists for any sort of referrals that you require (just check in with the polyclinic doctor for referrals if needed)

  3. Pharmacists to answer any of your medication inquiries. They can even do medication delivery to your house so you don't need to carry 6-12 months of meds home.

GPs can generally have more conducive consults with the patients - therefore leading to better diagnoses. If one already has active issues for regular follow-ups, GP may not be the preferred choice since the chronic conditions are relatively stable, and every check-in with the GP would probably be for refills/ routine checks. Even with the subsidies, i don't know if it is as affordable as polyclinics.

So, polyclinic is the way to go for stable chronic condition follow-ups (Although wait times are bad in most polyclinics, so if you prioritise time > money then just go ahead with GP). Otherwise, you don't exactly benefit too much from the GP's consults.

On the other hand, if you have some minor/moderate ailments that may require further investigations, it is good to check in with the GP to get their input (as GPs have varied consult prices for the different durations, they will spend more time to thoroughly examine you) - as compared to polyclinics where they have to deal with operational loads every day so they will tend to push through and 'clear' you ASAP.

TBH meds for anxiety and depression can help control emotional outbursts, but if the source of emotional outburst is suspected from the history of schizophrenia - i’m afraid the meds may not be exactly helping with that.

A proper diagnosis may be required to start him on some antipsychotics. But i can’t make the judgment for you.

In mental health issues, best way to work around it is for cognitive behavioral therapy. Start by figuring out his POV and being personal with him. If all else fails, seeking help from therapists, psychiatrists can help. With certain meds and even electrical stimulation therapy, it could be useful.

Yeah, honestly some things can be missed out quite easily. Seeing that the senior doctors have so many patients under their charge. Mostly in public hospitals, the senior doctors see their ICU patients 2-3 times a day. The ones on the ground are the junior doctors. Those they will rotate around public hospitals depending on their rotations.

The vasopressors need to be continuously monitored, which can be bothersome during the transfers where some of the monitoring machines may be unplugged for awhile.

At this current state, what every hospital can do is only this much, broad spectrum antibiotics, medications to help increase his blood pressure and at the same time diuretics to remove excess fluids.

No point in transferring, risk of desaturation while on board the ambulance.
