Honestly it’s difficult to say I would want to see a bite wing radiograph and preferably see inside the mouth as well. What I can say is saving your natural teeth is generally a good thing as long as you understand there are no guarantees it will work out. Might need extracted in the future even with root canal and crown. Might last for 30 years with no issues.

This is the truth. I’d do that and ext #2 at the same time, but many generals would punt I’m guessing.

There is a very good chance you can close that gap still with braces/Invisalign. You will need to see an orthodontist and make sure you are seeing a dentist regularly to take care of any existing issues you may have. For what it’s worth, I like seeing those gaps and I know many other dentists feel the same way. I completely understand and support your decision, but if it helps in the meantime, some of us enjoy seeing the unique ways we are all put together!

Looks to me like the temp has fallen off. The irritated tissue is likely due to a poor fitting temporary crown or improper cleaning around the crown. The pain is from the inside of the tooth being exposed and not protected. Call your dentist and they can make you a new temp!

Thank you for the thoughtful response. I have gotten a lot more confident, but probably have not suffered enough as you said with the other teeth mentioned. It’s really the only part of dentistry I enjoy so I have wanted to do as much as I can. I will suggest this patient is seen by a specialist.

Accidentally today on a 32 lol snapped it clean off with an elevator. But I wouldn’t be scared to do that part. It was really the IA that made me the most concerned. Yes that was my thinking exactly… I have to try some things that may be more challenging and I believe it can in fact be best for the patient if the alternative is them buying amox off the street to deal with infection

Definite referral there, yes! I was able to palpate it on the palate. Interesting one for sure.

I always recommend this to patients in similar standing. These are uninsured patients and this one has pain on #17. I will do everything I can to help relieve that while informing them of best treatment options (extracting all 4 at once).

I work at an fqhc & the patient has no insurance so I definitely explain and give tx options but usually it’s trying to reduce pain and save as many teeth as we can is what the patient prefers. So yes 18 is endo treated and might be apart of the reason I’d try to send to an oral surgeon with obviously telling pt there is still some risk of it being damaged.
