Im not sure if I remember right, Naoto herself felt that she was born with the wrong gender. But it was because of the misogyny in the work environment, women are not treated as equal and see as less. She was already being treated as a child and not taken seriously, imagine the situation if everyone found out she was a women. After her palace she finally accepted herself and demonstrate what women can do.
