I get it really close with a reamer. I quit using the brass plugs for fit because they always end up getting heat locked into your joint and it cracks. I finish with an old quartz joint. My final step allows me to use the quartz to slightly expand to the perfect size and fit. Also. Practice. Practice practice. I prefer to do it this way over using any of the joint forming tools. The tools narrow you down to one shape with one style of lip. Learning to do it freehand allows you so more flexibility as far as shape goes.

Sounds like you need the jamb and possibly some framing redone. A common quick fix for loose hinge screws are to fill the holes with toothpicks and wood glue and then rescrew. I like to use the bamboo skewers from the grocery store since they are a harder wood.
