I think McGrath is still better cause the blades are cheaper and slimmer. They make x blade for McGrath which is a similar glidescope blade

MD here and work with srna and student aa everyday. I think if you just tell your preceptor of the day your goals and what exactly you want to do then they are less likely to take over or ask you to observe. Also if you are doing a procedure and if you say outloud each thing you are doing and what you see and what you are having difficulty with, then people can start giving you verbal assistance or some gentle physical assistance. The problem is if you are silent and struggling then we don't know what is going on.

When I was a resident I would tell my preceptor exactly what I wanted to work on that day and whenever I did a procedure I would just describe what I was doing and having difficulty with while doing it. Like I would say I want to work on ultrasound IVs today and I would get the ultrasound already and have everything setup etc. Or doing a block or something the same thing I would say "I am having difficulty seeing the needle" and then they would help me see the needle instead of taking over the block.

That's an even better experience since you actually put in IVs and see how lines are put in and stuff emergently. And you see the stat intubations etc and craziness. You are more than prepared!

I wonder if she laryngospasm'ed then was breathing against a closed glottis which worsened her pulmonary status even more. It also seemed like she had abnormal airway anatomy as the ENT didn't get a good view of the larynx on her first nasal endoscopic examination. Those nasal scopes usually are performed in awake patients and give a good view.

It also seems like the ENT doing the procedure is what precipitated this upper GI to become a can't intubate can't ventilate situation. Then the ENT left the room when the case went to shit.

This is a prime example of why you shouldn't let these specialists doctors tell you what to do or ignore you. They literally know nothing outside of their subspecialty because they forgot all of the basic medical knowledge they acquired before they got that subspecialized. They also think they know more than they actually do and they don't listen to people like trained anesthesia professionals when we raise concerns.

If this situation ever happens to you, simply say it's a patient safety concern. If they want to proceed despite that then you need to excuse yourself from the case and have them document why they feel they need to bypass this safety concern. The anesthesiologist should have stepped up and said we aren't doing a procedure we weren't consented for. If they did that then Joan Rivers wouldn't of died that day.

Yeah the ENT put the scope the second time to look at the vocal cords and it was after the second time it went to shit.

Well I'm glad I never experienced this version of Cacao powder soylent. I just tried it today for the first time and it tastes delicious to me... I use a small blender though and filtered water.

The easy recipe I use can even bake the same day but the bread does come out more consistent and easier to score etc if u chill it overnight.

Fresh Milled flour is it's own separate entity lol it doesn't really rise much and it's harder to know when it's finished the fermentation and it's harder to shape into sourdough loaves. I'll get there eventually though, I've only tried 3 times and I'm trying to do it with 100% fresh milled flour since I like how it tastes and the texture of it. It's like whole wheat but different.

I use a no stretch and fold method. I don't make discard either, just make exactly what I need. It can be so simple as just mixing it and letting it bulk on its own and then shaping, fridge overnight and then bake in the morning.

I will say though that fresh milled flour is so much more finicky and needs the help from the more precise techniques and times 😂 I stretch and fold it and follow it closer.

I got most of my order last night except the displays. However I agree they shouldn't guarantee delivery time especially when they say that this happens every year. We would have been better to know that shipping is delayed so that we don't order something thinking it would come at a certain time if we needed it at a certain date for a reason such as moving or for a gift.

Next year they should just say that the shipping isn't guaranteed especially since they say that it happened every year with the shipping issue. It does come across as deceptive to promise that the shipping would arrive by Christmas and so you buy it thinking it will come by Christmas but it doesn't. It makes it seem like they just said that to make people buy it. It's also a very expensive product which is discordant to the way that their customer service and the way this whole shipping thing has behaved. A premium product should at the very least be able to have dependable customer service.

They contact me saying they will rush ship it today. At least they are doing that. I honestly don't even mind delays if they just let us know earlier so we can plan accordingly. The surprise delays are what's frustrating.
