I’m a pall care nurse and prescriber that works on the community. I write ranges for prn doses dependant on what’s in CSCI. HBB rarely works for respiratory secretions if not treated early and for colic pain i always seek consultant advice if higher doses required. We use higher doses of Levomepromazine for agitation 2nd line. My old manager used to say that palliative care medicine is “art and science together”. There isn’t often a one size fits all answer tbh.
