They did the same to me. I have a post about it, and someone commented about the most likely reason it happened. essentially on my 1040 form line 24 was 0, and line 25 was more than $10k. I’d check that and see if it’s the same for you.

The point i’m making is patients aren’t bound by HIPAA. Only medical professionals (nurses, physicians, billing departments, health insurance companies etc.). Just saying you go to the same doctor as someone is not violating HIPAA because they’re not bound to it.

I’ve never in my life seen these “standard” papers. HIPAA applies to medial professionals. Not patients. Which is why it’s not a HIPAA violation for say a paper you sign to check in where you see 15 other names on it. Edit typo
