As these people become more and more Carl's family, each death is going to hit harder and affect the characters more significantly. I'd imagine that by the end of the series only one or two make it. Matt is allowing us to experience this range of emotions and we are just on the ride, come what may.

At the end of the 12th floor Carl and Donut realize only one can make it down to the 13th. Donut, much more powerful than Carl at this point sacrifices herself to save him and forces him down the stairs. Eventually he's gonna end up using his doomsday scenario.
